The office is open from 8:00am- 5:00pm Monday through Friday. Patients are seen by appointment only. We will try to honor your scheduled appointment because we value your time. Please understand that medical emergencies do occur and in these circumstances we ask for your consideration.
If you cannot keep an appointment, we ask that at least 24 hours notice be given to the office. This makes it possible for us to give that appointment time to another patient. Patients arriving more than 20 minutes late and without notice will be asked to reschedule their appointment. If we are not notified 24 hours in advance there will be a $30.00 fee.
In case of a life threatening emergency, please call 911. Our office has an answering service which can contact the doctor for urgent messages. For refills on medications, please call your pharmacy.
Accepted Insurance Plans
Our practice accepts most major insurance plans and most of the smaller insurance plans. If we do not participate with your insurance and your plan allows out of network benefits, our office staff will assist you with the necessary paperwork so you can receive reimbursement directly from your insurance company.
If your insurance plan is not on this list, please call our office to confirm we are in network for your plan. Please have your insurance details when calling in to book your appointment to determine if we are in-network.

Eye Institute of Houston provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
Please complete the Patient Registration forms prior to your first visit. It is important that the information you provide be accurate since we will rely on this data to diagnose and treat you. Please advise us whenever there is a change of address, telephone number or insurance coverage.

This will also speed up the time that is takes for the technician to work you up.
Please bring this paperwork to your appointment. Do not mail the paperwork.
Please note: Your eyes may be dilated during the exam so you may want to plan on having someone with you to drive home.
